We are seeking a PRN Occupational Therapist to join our amazing team at Evergreen, a warm and welcoming Continuing Care Retirement Community. Responsibilities:

  • Conduct assessments and develop individualized treatment plans for residents to improve their physical, cognitive, and emotional well-being.
  • Implement therapeutic activities and interventions to support residents in performing daily living tasks and enhancing independence.
  • Collaborate with a multidisciplinary team to ensure coordinated care.
  • Provide education to residents and families on adaptive techniques, assistive devices, and safety measures.
  • Document and track resident progress, adjusting plans as necessary to meet goals.

What We're Looking For:

  • Licensed Occupational Therapist in the state of Wisconsin.
  • PRN availability—whether you’re looking for a few shifts or more!
  • Previous experience in long-term care, geriatrics, or rehabilitation preferred, but not required.
  • A passion for working with seniors and a commitment to promoting independence and dignity.
  • Strong communication skills and a positive, team-oriented attitude.

Working hours are typically 8:30am-5:00pm Monday through Friday and 1-2 on-call weekends a month. The hours for this position can be flexible.
