Social Worker

Location: Oshkosh, WI

Evergreen is seeking compassionate and talented Social Worker to join the Rehab Services Team!

Evergreen is a Christian, non-profit senior living organization serving older adults ages 55 and better. Located in a pleasant residential neighborhood in Oshkosh, Evergreen offers many options to meet unique needs.

Evergreen's 80-bed Medicare-certified licensed skilled nursing facility offers short-term rehabilitation care, long-term care and palliative care. Creekview long term skilled nursing provides care for 58 residents in six households with a nationally-recognized innovative approach. Creekview Rehabilitation Center at Evergreen provides care for 22 residents who are focused primarily on short-term rehabilitative therapies and who plan to return home after recovery.

The position is 8:00am-4:30pm Monday-Friday working 40 hours a week.

Starting wage based on experience is $24-$28/hr. and includes annual increase.


Job Duties

  • Assist residents/families/legal representatives with arrangements to ensure an efficient and orderly admission or discharge including paperwork, transportation, personal belongings, financial planning, needed referrals and emotional support and guidance.
  • Obtain insurance authorizations and provide insurance updates as indicated.
  • Coordinate resident room changes to meet the resident's best interests and needs.
  • Participate in weekly care planning, Medicare, and Leadership Team meetings providing current information on residents being reviewed.
  • Assist staff with interdisciplinary approaches for resident care and participate in problem solving regarding social, emotional and behavioral problems.
  • Keep current with job related information, including regulations, to effectively carry out responsibilities.
  • Serve as advocate and facilitator for residents and families regarding questions, concerns, or problems that may arise within the resident's living situation at Evergreen.
  • Complete assigned charting and documentation (pre-admission assessments, social histories, care plans, resident interviews, MDS's, assessments, progress notes, etc.) within specified time limits.
  • Identify and write care plan approaches related to social services and follow through on
    implementation.
  • Attend daily stand up meeting.

Qualifications

  • Bachelors Degree in Social Work from an accredited college or university
  • Valid state Social Work certification
  • One year of social work in a long-term care setting preferred
  • Excellent communication and problem solving skills
